Privacy Policy

Organization: Canvi Behavioral Health

Effective date: July 1, 2026

This document contains two separate parts: Part I explains how Canvi Behavioral Health handles information collected through its public website; Part II is Canvi Behavioral Health’s HIPAA Notice of Privacy Practices for protected health information maintained in connection with patient care. If the two parts conflict regarding protected health information, Part II controls.

Part I — Website Privacy Policy

Canvi Behavioral Health (“Canvi,” “Canvi Health,” “we,” “us,” or “our”) respects your privacy. This Website Privacy Policy explains what information may be collected when you visit www.canvihealth.com, submit an online inquiry, use a website form, or follow a link to a third-party service.

 

1. Scope

This policy applies to Canvi Behavioral Health’s public website and website-based communications. It does not replace the HIPAA Notice of Privacy Practices in Part II, and it does not control the privacy practices of the patient portal, scheduling platforms, social media services, maps, or other third-party websites linked from Canvi’s website.

 

2. Information We May Collect

Information you provide

  • Contact information, such as your name, email address, phone number, and preferred location.

  • Appointment or service inquiry information, including the service you are interested in, your preferred contact time, insurance information, and questions you choose to submit.

  • Feedback, ratings, survey responses, or testimonial information you voluntarily provide.

  • Communications you send to Canvi through email, website forms, or other channels.

 

Please do not use a general website form for medication questions, appointment changes, urgent clinical concerns, or emergencies. Use the patient portal or contact the appropriate Canvi office. If you are experiencing an emergency or may harm yourself or someone else, call 911 or call or text 988 in the United States.

Information collected automatically

 

When you use the website, Canvi and its service providers may automatically collect information such as your Internet Protocol address, browser type, device type, operating system, referring page, pages viewed, links clicked, approximate geographic area, date and time of access, and cookie or similar identifiers.

 

3. How We Use Website Information

  • Respond to inquiries and connect you with the appropriate Canvi location or service.

  • Provide, operate, secure, troubleshoot, and improve the website.

  • Understand website use, measure communications and advertising, and improve patient education and outreach.

  • Protect against fraud, misuse, security incidents, and unlawful activity.

  • Comply with legal, regulatory, contractual, and professional obligations.

  • Carry out another purpose described when the information is collected or that you authorize.

 

4. Cookies, Analytics, and Advertising

Canvi may use cookies, pixels, analytics tools, and similar technologies to keep the website functioning, remember preferences, understand traffic, measure outreach, and improve communications. Some tools may be provided by third parties, such as website hosting, analytics, search, or advertising providers.

Canvi does not intentionally upload protected health information to advertising platforms or authorize analytics or advertising providers to use information from patient portal or medical record systems for their independent advertising purposes. Advertising or marketing trackers are not intended to operate on authenticated patient portal pages or pages dedicated to detailed clinical intake. General informational pages may still generate technical data, such as an IP address, device identifier, or page-view event, that a service provider processes under its own privacy terms.

A cookie banner or preference setting is not a HIPAA authorization. Canvi evaluates tracking technologies under applicable privacy and health-information laws and does not intend for general analytics or advertising tools to collect the contents of clinical communications or information entered into patient portal systems.

You may be able to control cookies through a website preference tool, where available, and through your browser or device settings. Blocking cookies may affect some website functions.

 

5. How We May Disclose Website Information

  • To vendors and service providers that support website hosting, forms, communications, analytics, security, scheduling, or other business functions, subject to appropriate contractual and legal safeguards.

  • To health plans, providers, or other parties when you request or authorize the disclosure or when needed to respond to your inquiry.

  • To comply with law, legal process, licensing requirements, court orders, or lawful government requests.

  • To protect the rights, safety, security, and property of Canvi, its patients, website users, or others.

  • In connection with a merger, reorganization, financing, acquisition, or transfer of business assets, subject to applicable privacy law.

 

Canvi does not sell protected health information. Canvi also does not sell personal information collected through general website forms for money.

 

6. Health Information and HIPAA

Information you submit through the public website is not automatically protected health information in every circumstance. However, when Canvi receives or maintains identifiable information in connection with treatment, payment, or health care operations, that information may become protected health information and will be handled as described in Part II and applicable law.

The patient portal is the preferred method for secure clinical communications and access to medical information. Email and ordinary website forms may not provide the same level of security as the patient portal.

 

7. Your Choices and Privacy Requests

  • You may choose not to submit information through the website and may contact a Canvi office by phone instead.

  • You may unsubscribe from nonessential marketing emails by using the unsubscribe link or contacting Canvi.

  • You may adjust browser or device settings to limit cookies and similar technologies.

  • You may request access, correction, or deletion of website information, subject to applicable law and Canvi’s record-retention obligations.

  • Requests involving medical records or protected health information are governed by Part II.

 

Residents of some states may have additional rights regarding personal information, depending on the law and whether it applies to Canvi or the information involved. Canvi will not discriminate against you for making a privacy request where prohibited by law. To submit a website privacy request, contact admin@canvihealth.com.

 

8. Data Retention

Canvi retains website information for as long as reasonably necessary for the purposes described in this policy, to respond to inquiries, maintain appropriate business and clinical records, comply with law, resolve disputes, and protect the security and integrity of its systems. Retention periods vary according to the nature of the information and applicable requirements.

 

9. Security

Canvi uses reasonable administrative, technical, and physical safeguards designed to protect information. No website, email, transmission, or storage system can be guaranteed completely secure. Do not send sensitive clinical information through ordinary email or a general website form unless Canvi specifically directs you to do so.

 

10. Children and Minors

The public website is intended for a general audience and is not designed for children under 13 to use independently. Canvi may provide clinical services to minors with appropriate involvement of a parent, guardian, or other authorized person, consistent with applicable law and the minor’s privacy rights.

 

11. Third-Party Services and Links

The website may link to a patient portal, maps, social networks, scheduling systems, educational resources, or other third-party services. Canvi is not responsible for the content, accessibility, security, or privacy practices of third parties. Review the terms and privacy notices of those services before providing information.

 

12. Changes to This Website Privacy Policy

Canvi may update this policy as its website, services, vendors, or legal obligations change. The updated policy will be posted on the website with a revised effective date. Material changes will apply prospectively unless otherwise permitted or required by law.

Part II — HIPAA Notice of Privacy Practices

YOUR INFORMATION. YOUR RIGHTS. OUR RESPONSIBILITIES.

 

Effective date: July 1, 2026.

This notice applies to Canvi Behavioral Health, its participating providers, workforce members, and the Canvi locations listed in this document when they use or maintain protected health information in connection with patient care.

 

Summary of Your Rights

  • Get an electronic or paper copy of your medical record.

  • Ask Canvi to correct your medical record.

  • Request confidential communications.

  • Ask Canvi to limit certain uses or disclosures.

  • Get a list of certain disclosures of your information.

  • Get a paper or electronic copy of this notice.

  • Choose someone to act for you.

  • File a complaint if you believe your privacy rights were violated.

 

Your Rights in Detail

Get a copy of your medical record

You may ask to see or receive an electronic or paper copy of your medical record and other health information Canvi maintains about you. Canvi will provide a copy or summary as required by law, generally within 30 days, and may charge a reasonable, cost-based fee where permitted.

 

Ask Canvi to correct your record

You may ask Canvi to correct health information you believe is incorrect or incomplete. Canvi may deny the request in some circumstances but will explain the reason in writing, generally within 60 days.

 

Request confidential communications

You may ask Canvi to contact you in a specific way or at a different address. Canvi will accommodate reasonable requests.

 

Ask Canvi to limit uses or disclosures

You may ask Canvi not to use or disclose certain information for treatment, payment, or health care operations. Canvi is not always required to agree. If you pay in full out of pocket for a service, you may ask Canvi not to disclose information about that service to your health plan for payment or health care operations, unless disclosure is required by law.

 

Receive an accounting of disclosures

You may request a list of certain disclosures Canvi made during the six years before your request. The list generally will not include disclosures for treatment, payment, or health care operations or certain disclosures you authorized. One accounting in a 12-month period is provided without charge; a reasonable, cost-based fee may apply to additional requests.

 

Get a copy of this notice

You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.

 

Choose a personal representative

If someone has legal authority to act for you, such as a health care power of attorney or legal guardian, that person may exercise your rights after Canvi verifies the authority.

 

File a complaint

You may complain to Canvi or to the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights were violated. Canvi will not retaliate against you for filing a complaint.

 

Your Choices

For certain health information, you may tell Canvi your preferences about disclosure. If you are unable to communicate your preference, Canvi may disclose information when it is in your best interest or necessary to lessen a serious and imminent threat to health or safety, as permitted by law.

  • Family, friends, and others involved in your care or payment: You may tell Canvi whether and how to share information.

  • Disaster relief: You may tell Canvi whether to share information with an organization assisting with disaster relief.

  • Marketing, sale of information, and most uses of psychotherapy notes: Canvi generally must obtain your written authorization.

  • Fundraising: If Canvi contacts you for fundraising, you may tell Canvi not to contact you again.

 

How Canvi May Use or Disclose Health Information

 

Treatment

Canvi may use your information and share it with other health professionals who are treating you or coordinating your care.

 

Health care operations

Canvi may use and disclose your information to operate the practice, improve care, train staff, conduct quality assessment, manage services, and contact you when necessary.

 

Payment

Canvi may use and disclose your information to bill and obtain payment from health plans or other responsible parties.

 

Business associates

Canvi may share information with vendors that perform services for Canvi when agreements and applicable law require them to safeguard the information.

 

Appointment reminders and health-related services

Canvi may contact you about appointments, treatment alternatives, or health-related benefits and services that may interest you, as permitted by law.

 

Other Uses and Disclosures Permitted or Required by Law

  • Public health and safety activities, including disease prevention, product recalls, adverse-event reporting, suspected abuse or neglect, and preventing or reducing a serious threat to health or safety.

  • Health research when legal and ethical requirements are satisfied.

  • Compliance with federal or state law and lawful oversight by the U.S. Department of Health and Human Services.

  • Organ and tissue donation requests.

  • Coroners, medical examiners, and funeral directors when an individual dies.

  • Workers’ compensation claims, law enforcement purposes, health oversight, and special government functions when legally authorized.

  • Court or administrative orders, subpoenas, and other legal proceedings when applicable requirements are met.

 

Additional Protection for Certain Records

Certain records may receive additional protection under federal or state law, including psychotherapy notes, some mental health records, substance use disorder records subject to 42 CFR Part 2, and other specially protected information. Canvi will follow the more protective law when it applies. Where Part 2 applies, records generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your written consent or a qualifying court order and subpoena.

 

Canvi’s Responsibilities

  • Maintain the privacy and security of your protected health information.

  • Notify you promptly if a breach occurs that may have compromised the privacy or security of your information, as required by law.

  • Follow the duties and privacy practices described in the notice currently in effect.

  • Not use or disclose your information other than as described in this notice unless you authorize it in writing. You may revoke an authorization in writing, except to the extent Canvi has already acted in reliance on it.

 

Changes to This Notice

Canvi may change the terms of this notice. A revised notice may apply to all protected health information Canvi maintains, including information created or received before the revision. The current notice will be available on Canvi’s website, at Canvi offices, and upon request.

 

Privacy Questions, Requests, and Complaints

Contact Canvi’s Privacy Officer at admin@canvihealth.com. Written requests may also be sent to the Canvi location most relevant to your care. Canvi will not retaliate against you for making a request or filing a complaint.

 

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by visiting https://www.hhs.gov/hipaa/filing-a-complaint, calling 1-877-696-6775, or writing to 200 Independence Avenue SW, Washington, DC 20201.
