These Website Terms & Conditions (“Terms”) govern your access to and use of www.canvihealth.com and its public-facing content, forms, and links. By using the website, you agree to these Terms. If you do not agree, do not use the website.
1. About This Website
Canvi Behavioral Health provides general information about behavioral health conditions, services, providers, locations, educational resources, and ways to contact Canvi. Website content is provided for general informational and educational purposes.
2. Not Medical Advice
Website content is not medical advice, diagnosis, treatment, or a substitute for consultation with a qualified health care professional. Do not start, stop, or change medication or treatment based solely on website information. Treatment recommendations depend on an individual clinical evaluation.
3. Emergencies and Crisis Support
Do not use this website for emergencies!
If you are experiencing a medical or mental health emergency, may harm yourself or someone else, or need immediate help, call 911. In the United States, you may also call or text 988 to reach the Suicide & Crisis Lifeline. Website forms, email, and social media are not monitored continuously and are not emergency services.
4. No Provider–Patient Relationship
Using the website, reading content, submitting a general form, or sending an email does not by itself create a provider–patient relationship. A treatment relationship is established only through Canvi’s clinical intake and acceptance process and appropriate professional evaluation.
5. Appointments, Inquiries, and the Patient Portal

An online appointment or consultation request is not confirmed until Canvi contacts you and confirms it.

Do not use general website forms for medication questions, prescription refills, appointment changes, urgent clinical concerns, medical records, or referrals unless the form specifically permits that use.

Use the patient portal or contact the appropriate Canvi office for secure clinical communications.

Email and ordinary website forms may not be encrypted or secure. Avoid including unnecessary sensitive information.

Information about insurance coverage, eligibility, availability, or cost is an estimate until verified. Benefits and authorization requirements are determined by the applicable health plan.

Telehealth and Location Requirements
Canvi may offer telehealth services when clinically appropriate. Telehealth care is available only when the patient is physically located in a jurisdiction where the treating professional is licensed and authorized to practice at the time of the appointment. You must provide accurate location information and comply with any identity, consent, technology, safety, and emergency-planning requirements. Canvi may reschedule, redirect, or decline a telehealth visit when legal, clinical, licensing, or technical requirements are not satisfied.
7. Communications
If you provide an email address or phone number, Canvi may use it to respond to your inquiry, provide scheduling or administrative information, and communicate about services you requested, consistent with your preferences and applicable law. Email and text messages may not be secure; message and data rates may apply. You may request that nonessential communications stop, but Canvi may still send treatment, payment, safety, or legally required communications when permitted.
8. Minors
The public website is not designed for children under 13 to use independently. A parent, guardian, or legally authorized person should assist minors with general website inquiries. Clinical consent, confidentiality, record access, and parental involvement are governed by applicable law and the circumstances of care.

Canvi grants you a limited, revocable, nonexclusive right to access and use the website for personal, noncommercial purposes. Third-party trademarks and content remain the property of their respective owners.
11. Information and Feedback You Submit
You are responsible for information you submit and represent that you have the right to provide it. Do not submit unlawful content or confidential information belonging to another person without authority. If you submit feedback, a rating, or a testimonial, Canvi will obtain any permission required before publicly using identifiable information. Submission does not guarantee publication or response.
12. Third-Party Services and Links
The website may link to a patient portal, maps, scheduling tools, social networks, payment or insurance resources, educational materials, and other third-party services. Those services are governed by their own terms, privacy notices, security practices, and accessibility features. Canvi does not control and is not responsible for third-party content or services. A link does not necessarily imply endorsement.
13. Privacy
Canvi’s collection and handling of personal information is described in the Website Privacy Policy and HIPAA Notice of Privacy Practices. Those documents are incorporated into these Terms by reference. If these Terms conflict with the HIPAA Notice regarding protected health information, the HIPAA Notice controls.
14. Accessibility Statement

Canvi Behavioral Health is committed to providing a website that is accessible to the widest possible audience, including people with disabilities. Canvi is working toward alignment with the Web Content Accessibility Guidelines (WCAG) 2.2 Level AA and treats accessibility as an ongoing responsibility.

Known limitations and third-party content
Some third-party tools, embedded content, patient portal functions, maps, social media features, or older website content may not yet provide the same level of accessibility as Canvi’s primary website. Canvi does not control every third-party platform but welcomes reports of barriers and will work to provide information or assistance through another reasonable method when possible.

Canvi does not guarantee that the website will always be available, uninterrupted, current, complete, or free from errors. Information about providers, services, locations, insurance, and availability may change.

To the fullest extent permitted by law, the website and its content are provided “as is” and “as available.” Canvi disclaims warranties of any kind, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose, noninfringement, security, and uninterrupted availability. Nothing in these Terms limits duties Canvi owes to established patients under applicable professional or health care law.
17. Limitation of Liability
To the fullest extent permitted by law, Canvi and its owners, affiliates, providers, employees, contractors, and agents will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages arising from or related to use of or inability to use the website, reliance on website content, or third-party services. These limitations do not apply where prohibited by law and do not limit liability that cannot legally be excluded.
18. Indemnification
To the extent permitted by law, you agree to defend, indemnify, and hold harmless Canvi and its affiliates, owners, providers, employees, contractors, and agents from claims, losses, liabilities, costs, and expenses arising from your unlawful misuse of the website, violation of these Terms, or infringement of another person’s rights.
19. Governing Law and Venue
These Terms are governed by the laws of the State of Idaho, without regard to conflict-of-law principles, except where federal law or a nonwaivable law of another state applies. Any dispute relating solely to website use will be brought in a court with jurisdiction in Bannock County, Idaho, unless applicable law requires another forum. Canvi’s Utah services remain subject to applicable Utah health care, licensing, privacy, consumer-protection, and professional-practice laws. This section does not govern professional-liability, insurance, employment, or patient-care disputes that are subject to separate agreements or legal requirements.
20. Changes to These Terms
Canvi may revise these Terms from time to time. Updated Terms will be posted on the website with a revised effective date. Your continued use of the website after an update constitutes acceptance of the revised Terms to the extent permitted by law.
21. Severability and No Waiver
If any provision of these Terms is found unenforceable, the remaining provisions will remain in effect and the unenforceable provision will be interpreted as closely as permitted to its original purpose. Canvi’s failure to enforce a provision is not a waiver of that provision or any other right.
